Immunizations
Before your child attends school, you must show proof of adequate immunization from a doctor or clinic:
4 DPT doses (diphtheria, pertussis, tetanus – one after the 4th birthday)
4 Polio doses (one after the 4th birthday)
3 Hepatitis B doses (appropriate spacing)
2 MMR doses (measles, mumps, rubella)
2 Varicella doses (chicken pox)
Seventh through 12th Grade ADDITIONAL immunization requirements for attendance:
Entrance into 7th Grade:
1 Tdap dose (tetanus, diphtheria, acellular pertussis)
1st dose of MCV (meningococcal conjugate vaccine) for Meningitis
Entrance into 12th Grade:
2nd dose MCV (meningococcal conjugate vaccine) for Meningitis

First dose is given 11-15 years of age; a second dose is required at age 16 or entry into 12th grade.

If the dose was given at 16 years of age or older, only one dose is required.
If your child is exempt from immunizations due to religious or medical reasons, your child may be removed from school during a disease outbreak.
**A physical examination is required upon original entry into school and again in 6th and 11th grades.
**A dental examination is requiredupon original entry into school and again in 3rd and 7th grades.
